Grant Request Form
The University of Maryland Panhellenic Association budgets for each PHA chapter to receive a $400.00 grant per academic year. A chapter may only apply for a grant once per academic year. A grant can only be used for programming purposes that can be beneficial to the chapter or others on campus. The grant cannot be used for food, decorations, or to fund a philanthropy event. Please submit grant applications at least three weeks prior to the expected date of the event.

Hazing Concern Form
This form can be filled out by any concerned member of the University of Maryland community regarding incidences of hazing. Disclaimer: PHA does not directly investigate incidents. Information will be passed along to the appropriate persons.
